Compared to last year, the event organised by the Automobile Club Sassari moves forward by a week and will be run on Saturday 15 and Sunday 16 April, with the locality of Porto Cervo once again as the fulcrum of the event which, repeating the novelty of 2022, also proposes the Martini Rally Vintage, a non-competitive rally dedicated to cars bearing the historic livery.
A few changes in the route are announced, maintaining however the configuration with the eight special stages equally divided into two legs, with the first one offering the "Aglientu" in the extended version and just under 18 kilometres long; always in the first leg there will be, in the final phase, a regrouping in Piazza della Chiesa in San Pantaleo during which the crews will be invited to an aperitif at the end of the stage. The total timed kilometres will be just over 110 out of the race's total 450. For the regularity sport, following the new regulations, there will be six pressure tests and two with the average formula, while for the average race, which will kick off the 2023 edition of the Italian Championship, as many as thirty-two secret stages are scheduled for each leg.

Over the years, the Rally Costa Smeralda has been one of the benchmark events on the international rally scene.
Strongly desired by Prince Karim Aga Khan and the then President of the ACI Sassari Marquis Don Franco di Suni as a vehicle event to promote the Costa Smeralda, charm, beauty, class, worldliness and glamour were all distinctive elements of the event in which organisers, drivers, fans and sponsors recognised the value and importance of the race.
The first edition took place in April 1978 and in 2018, on the occasion of its 40th anniversary, the race became the International Historic Rally once again. Rally history was born here and champions of the calibre of Verini, Toivonen, Kankkunen, Cerrato and Biasion, some of whom went on to become World Champions, won here.
